Recovery after breast revision: what to expect, week by week

Revision recovery often differs from primary surgery — and from one revision type to another. Here is what's typically faster, what's slower, and what's the same.

Revision recovery in context

The most common patient assumption is that revision recovery will be harder than the original surgery. In most cases this is not true. For pocket adjustments, simple implant exchanges, and smooth size changes, recovery is often shorter than primary augmentation — the soft tissue is already accustomed to implants and the muscle (if implants were submuscular) doesn't need to re-stretch.

For capsulectomy with implant exchange or explant with concurrent lift, recovery is comparable to primary surgery — sometimes slightly longer because of the additional dissection.

Drains are common after capsulectomy (typically 3–5 days). Compression garments are used for 4–6 weeks. The general activity restrictions are similar to primary augmentation.

Week-by-week timeline

Day of surgery

Typical operating time 2–5 hours under general anaesthesia. Hospital observation either same-day discharge or one overnight depending on complexity. Walking encouraged within 4–6 hours of surgery for DVT prevention.

Days 1–3

Most painful period. Discomfort is well-managed with prescribed analgesics. Compression garment or surgical bra worn continuously. Sleeping on back, head slightly elevated. Walking around the room, no lifting.

Week 1

Drain check daily if drains placed. Pain decreases substantially by day 4–5. Light walking outside hotel, gentle daily activities. First in-person follow-up with Dr. Erdal at 24h, 72h, and day 5–7. Drain removal if applicable.

Weeks 2–3

Most patients return home from Istanbul during this period. Office work resumes for most procedures. Compression garment or supportive bra continues. No upper-body resistance exercise. Sleeping back-elevated continues.

Weeks 4–6

Swelling decreases noticeably. Implants begin "settling" into final position. Light cardio (walking, stationary bike) is permitted. Resistance training still restricted. Sleeping on side may be allowed — surgeon-specific timing.

Weeks 6–8

Most patients return to full exercise including upper-body strength training. Final shape becomes apparent. Scars are pink and firm — full maturation takes 12 months.

3–6 months

Final aesthetic outcome. Scars continue to fade. Sensation normalises. Soft tissue around implants stabilises in final position.

What to pack for Istanbul

  • Front-zip or front-button tops — pulling shirts over your head is uncomfortable for the first 2 weeks.
  • Comfortable elastic-waist trousers and slip-on shoes — bending is restricted.
  • Two surgical bras — supplied by the clinic, but bring an extra for backup.
  • Loose pyjamas — for hotel recovery.
  • Phone charger and entertainment — you'll spend a lot of time resting.
  • All medications you currently take with original packaging.
  • Operative records, implant card, photos — bring originals for consultation reference.

Warning signs — when to call the clinic

Most patients have uneventful recoveries. The following signs warrant immediate contact:

  • Sudden, significant unilateral swelling or bruising
  • Asymmetric breast firmness developing days after surgery
  • Fever above 38.5°C (101.3°F)
  • Severe sudden pain not relieved by prescribed medication
  • Calf pain or shortness of breath (DVT/PE warning signs)
  • Unusual drainage from incision (cloudy or foul-smelling)
  • Wound separation
  • Visible deformity that wasn't there at discharge

The clinic provides 24/7 contact for the first 6 weeks post-operatively. After-hours contacts go directly to Dr. Erdal or his coordinator.

Follow-up schedule

While in Istanbul

  • 24 hours post-op (in person, by Dr. Erdal)
  • 72 hours post-op (in person)
  • Day 5–7 (in person, before discharge to travel)

After return home

  • Week 2 (video call)
  • Week 6 (video call with photos)
  • Month 3 (video call with photos)
  • Month 6 (video call with photos)
  • Month 12 (video call with final outcome photos)

All follow-up is included in the original quote. Complication management within the first 6 months — including any unplanned operation, hospital stay, or surgical revision of the revision — is included if performed at the same JCI hospital.
